If you are a Physician who wishes to refer a patient to Lakeland Health Services for a sleep study, you will find that we have a simple process. Lakeland Health Services takes referrals directly from Primary Care Physicians. If you are confident that you know which type(s) of sleep study is required, simply fax your referral to us along with as much pertinent patient history as possible to allow us to better serve the patient. In this history, please include the patient’s symptoms as well as general important information such as allergies and conditions that we should be aware of. If you are not sure that a sleep study is appropriate for a particular patient or which study to order, you may send us a referral that indicates you would like this patient to first have a consult with one of our Sleep Physicians. They will then order the appropriate study and send you a report of the consultation. In either case, the sleep study is interpreted by one of our Sleep Physicians. This Sleep Physician will then see this patient within 2 – 3 weeks for a follow-up. You will receive a copy of the interpretation and follow-up dictation. You may designate which of our Physicians performs the interpretation and follow-up. In some cases, you may even follow-up with your own patient regarding the sleep study if you wish.
